Medication Assisted Treatment (MAT)

Medication Assisted Treatment consists of medical visits with a certified physician to determine your diagnosis and to manage your treatment. Behavioral health practitioners such as our counselors and case managers will provide behavioral health visits.

After being diagnosed you will see your doctor, counselor, and case managers on a regular basis as they provide your treatment. All of these services are ways to help you stay on the path to recovery and manage the challenges along the way.

Targeted Case Management (TCM)

Targeted case management (TCM) services are defined as services that will advocate for and assist the targeted population (adults with substance use disorder) in gaining needed medical, educational, social and other support services.

Individual Substance Use Counseling

Substance use disorder stems from issues beyond physical dependence. The multidisciplinary team will help clients learn how to cope with stress, triggers, and psychosocial development. Treatment plans will be individually customized to meet each client’s unique needs.

Group Substance Use Counseling

Group meetings will be held on a weekly basis with a variety of topics including relapse prevention, chemical dependency, parenting, domestic violence, anger management, etc. In groups you will find people struggling with cravings and old behavior patterns just like yourself. Lead by the counselor you will all contribute to the discussion about how to best avoid triggers and keep moving toward sobriety.

Behavioral & Medical Comorbidities

Substance use disorder is highly comorbid with other mental and medical conditions. At 2nd Chance, we take a comprehensive approach to treating these disorders concurrently. The multidisciplinary team will assess the need to refer clients to other providers for comorbidities diagnose and treatment.

Certified Peer Support Services

​​Peer support services are delivered by individuals who have common life experiences with the people they are serving. People with mental and/or substance use disorders have a unique capacity to help each other based on a shared affiliation and a deep understanding of this experience. In self-help and mutual support, people offer this support, strength, and hope to their peers, which allows for personal growth, wellness promotion, and recovery.

DUI Services

DUI Class and Assesments Image

​​2nd Chance Clinic is licensed and certified by the Division of Behavioral Health to offer Driving Under the Influence(DUI) assessments and education programs for persons convicted of DUI.

All patients are subject to an assessment prior to beginning the DUI education program.
